Services - Cardiovascular Program
 At American Choice Home Health Care, our care team assists each cardiac patient to return to their optimal state of health.
The Care Team may include:
- Registered Nurses who specialize in Cardiovascular care
- Physical or Occupational Therapists
- Medical Social Workers
- Diet and Nutrition Specialists
- Home Health Aides
Each visit, the patient receives a comprehensive assessment and education geared to their learning style. Working with the direction of your cardiologist and cardiac surgeon, our knowledgeable team will teach the patient and their family:
- Diet (Mediterranean, no added salt, low fat, low cholesterol)
- Cardiac medications
- Exercise and life style changes
By reducing post op infections and preventing readmissions to the hospital, we help the patient heal in an environment they know.
